Like most small South African towns Fouriesburg was once a study in inequality. Across the valley in Mashaeng a teenager walks the dusty road home from school. The inequalities, however, are not nearly as marked now as poverty has become the condition of the whole town. Rural farming communities are under pressure in South Africa with the uncertainties in the agricultural sector compounded by rapid urbanisation.

    Description: A ROAD LESS TRAVELLED: Boundaries in Dr J. V. Larsen's Career. A young mother to be waits to be examined in Dr Larsen's consulting room.Over the past decade my father saw increasing incidence of HIV/AIDS among young women in his clinics. The occurance of the disease is measured by the incidence among pregnant women. By the late 1990s KwaZulu Natal province had one of the highest HIV infection rates in the world. Although there have been reports of the incidence of the disease abating, the reality on the ground has been different. Even among people well informed about the disease, behaviour change in terms of sexual practices has been slow in coming. The advent of the disease for me was marked by my father starting to wear large goggle-like glasses. My father once cut himself while operating on an HIV positive woman and had to go on a course of antiretrovirals himself. Then, unwise decisions made by the new administration dismantled the structure. The regional perinatal and maternal mortality rates again climbed in the wake of those decisions. Years later the Department is attempting to reinstate the systems my father and other colleagues had put in place. Although officially retired, my father continues to serve, this time in a new sphere of medicine. He was asked to assist in a clinic and hospice for people living with HIV/AIDS in Howick, the town where he has retired.
